
Child and Teen checkups (C&TC) is the name of Minnesota's Early and Periodic Screening, Diagnosis and Treatment (EPSDT) Program. C&TC is a comprehensive health benefit for children and teens from newborn through the age of 20 who are enrolled in Medical Assistance.
Our goal is to keep children healthy!
Child and Teen Checkups include:
- Information about good physical health
- Information about thoughts, feelings and relationships with others
- Questions and answers about your child's health
- A complete physical exam
- Immunizations (shots)
- Hearing check
- Vision check
- Lab tests
- Monitoring growth and development
- Referral to the dentist
Use this health check for Head Start, WIC, School or Daycare.
Be sure to bring all the forms you need with you to the Child and Teen Checkup.
