Community Health Workers (CHW) come from the communities they serve, building trust and vital relationships with the purpose of improve health and lower health disparities. CHWs promote preventive health care and link individuals to health care coverage. CHWs help individuals and families effectively navigate through health and social service systems, improve the quality and cultural competence of care, increase health knowledge, and help build individual and community capacities.
